Why do people produce poor quality?

In your opinion, what all could be the reasons of making a poor product or service? Don't you think, all people would like to do a good job and produce good quality? But still you have a lots and lots of poor quality products.
Brainstorm a little bit, and add to the list of reasons, why people produce poor quality.

-Lack of Quality Standards
-Lack of Monitoring Techniques
-Lack of Conflicting Standards
-Lack of Work Methods
-Lack of Tools and Equipment
-Lack of Motivation
-Improper Requirments
-Lack of Communication
-Improper design
-Improper coding
-Improper testing
